Job Description
Title: SNF - Rehabilitation

Schedule: Day shift

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 38

Requirements:
- Valid CA nursing license
- Experience in Skilled Nursing Facility (SNF) settings
- Knowledge of NetHealth (Rehab Optima) preferred

Roles & Responsibilities:
- Provide patient care and support for individuals in the rehabilitation setting.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Monitor patient progress and adjust care plans as necessary.
- Maintain accurate documentation in compliance with healthcare regulations.
- Educate patients and families about care plans and rehabilitation goals.
- Support the discharge planning process for patients transitioning from rehabilitation services.
