Dietician (Dialysis Department)

Job Title: Dietician (Dialysis Department) Facility: Alliance Care Rehabilitation and Nursing Center Location: Irvington, New Jersey Shift: Part-Time, 9:00 AM - 5:00 PM Salary: $40 - $42 per hour
Responsibilities:
Complete a monthly Nutrition Assessment for all patients.
Develop individual care plans with tailored nutritional guidelines.
Visit and document all dialysis patients at least monthly, reviewing nutrition care based on lab results and changes in physician status.
Deliver counseling services to patients and their families, obtaining feedback as necessary.
Maintain detailed documentation in the Electronic Medical Record (EMR).
Attend team meetings and participate in interdisciplinary reviews of patient progress.
Develop Quality Assurance/Performance Improvement (QA/PI) nutritional initiatives and contribute to regular performance meetings.
Educate pre-End Stage Renal Dialysis patients on nutritional needs.
Provide in-service training to dialysis team members.
Requirements:
BA/BS in Nutrition or a related field.
Registered Dietitian (RD) credential required.
Minimum of 12 months experience as a Registered Dietitian, as mandated by State regulations.
Experience as a renal dietitian is preferred.
Strong PC skills.
Ability to assess patient status and take appropriate actions.
Capable of working independently with minimal supervision.
Exceptional interpersonal and communication skills, with the ability to interact effectively with individuals from diverse backgrounds and various organizational levels.
