Position Summary

We are seeking a caring, motivated, and organized SCN Screening & Navigation Specialist to help individuals access the support and services they need. As one of the first people members interact with, you will play a key role in understanding their needs, determining program eligibility, and guiding them toward resources that can improve their health, stability, and overall well-being.

• Conduct screenings to evaluate eligibility for SCN programs and services.

• Collect and review demographic, health, insurance, and social needs information.

• Identify challenges that may impact a member's health and quality of life, including housing instability, food access, transportation barriers, unemployment, behavioral health concerns, and other social determinants of health.

• Guide eligible individuals through the enrollment process.

• Connect members with available services and community resources.

• Facilitate access to transportation assistance, nutrition programs, personal support services, health education, and other supportive benefits.

• Ensure members understand their available options and next steps.

• Maintain complete and accurate member records.

• Ensure all documentation complies with SCN and Department of Health guidelines.

• Monitor outstanding assessments and follow up to obtain missing information.

• Support the development of personalized service plans.

• Partner with care managers, healthcare providers, behavioral health programs, hospitals, and community organizations to coordinate services.

• Monitor member participation and service utilization.

• Conduct outreach and provide support when members encounter barriers or require additional assistance.

• High School Diploma or equivalent required.

• Strong communication, listening, and relationship-building skills.

• Ability to conduct assessments and gather information with accuracy and sensitivity.

• Excellent organizational and time-management abilities.

• Self-motivated and able to work independently.

• Bilingual in English and Spanish or English and Yiddish required.

• Experience in healthcare, social services, case management, community outreach, or a related field is a plus.
