RN Specialty Nurse Navigator - Resource - Torrey Pines Clinic

Description This is a Full‑Time benefited position. Position works 4 10‑hour shifts, with one being remote. Position does not require weekends or holidays. Located at Scripps Green Torrey Pines Clinic, with occasional site travel as needed. We invite you to join our highly engaged team of Specialty Nurse Navigators. Responsibilities Navigating patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient. Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self‑reliance. Provide in‑person clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology/oncology clinic sites, as needed. Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. Providing disease and treatment specific education tailored to meet patient's individual goals of care. Onsite rounding throughout treatment plan implementation as needed to provide needed in‑person patient evaluation. Provide direct, hands‑on nursing care, as needed. Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self‑reliance. Assist patients with social services needs by identifying resources in the community and referring to these programs. Collaboration with system‑wide authorizations team, scheduling in other departments, as needed to ensure patient receives timely and comprehensive care. Qualifications Required Education/Experience/Specialized Skills: BSN plus 3 years' recent clinical nursing experience in a variety of clinical settings. Required Certification/Registration: CA RN License BLS if working in a clinical setting ONS Chemotherapy and Biotherapy Preferred Education/Experience/Specialized Skills/Certification: MSN OCN if working in Hematology, ONN‑CG if working in Oncology area Ambulatory experience preferred Minimum of two years of outpatient experience managing the care of patients in oncology or a related specialty Benefits and EEO Statement Scripps Health is an equal opportunity employer.
