Join Our Team as a Wellness Nurse (RN/LPN)
We are looking for a dedicated and empathetic Wellness Nurse (RN/LPN) to become a part of our Healthcare & Medical Services team within our assisted living and memory care community. This position is essential in ensuring the well-being of our residents.
Key Responsibilities- Conduct assessments of residents' health and wellness.
- Administer medications and provide necessary treatments.
- Develop and coordinate personalized care plans.
- Work collaboratively with physicians, families, and care staff to ensure comprehensive care.
- Promote resident health through education and monitoring of any changes in their condition.
- Encourage safety and well-being among residents.
- Current RN or LPN license.
- Compassionate approach to patient care.
- Strong communication and teamwork skills.
- Ability to work effectively in a collaborative environment.
