Job Description Title: Primary Care Nurse Practitioner (NP) Location - Albany Region, NY Pay: (Based on experience) + Benefits Flexible Schedule / Part - time / Full - time Schedule Options Part-Time 2–3 full days per week Travel between Hoosick Falls, NY and Cambridge, NY Full-Time 4–5 full days per week Travel between Hoosick Falls, NY, Cambridge, NY, and Troy, NY Job Overview We are seeking an experienced Nurse Practitioner (NP) to provide primary care services to residents in assisted living facilities throughout the Albany, New York region. This role offers flexible scheduling with both part-time and full‑time options and is ideal for providers who enjoy building long‑term relationships with geriatric patients. Responsibilities Provide comprehensive primary care services to assisted living residents. Perform patient assessments, diagnoses, treatment planning, and follow‑up care. Manage chronic conditions and preventative healthcare needs. Collaborate with facility staff, families, and interdisciplinary healthcare teams. Maintain accurate and timely clinical documentation. Develop meaningful, long‑term relationships with residents and caregivers. Required Qualifications Active New York State Nurse Practitioner license. Active DEA registration. Currently enrolled and credentialed with both Medicare and Medicaid. Primary care experience required. Nurse Practitioners must meet New York State independent practice requirements (minimum 3,600 clinical practice hours). Preferred Qualifications Geriatric care experience. Assisted Living, Long-Term Care (LTC), or Skilled Nursing Facility (SNF) experience. Compensation & Benefits Competitive compensation package. $5,000 Retention Bonus after 90 days.
