Elderplan and HomeFirst, Medicare/Medicaid managed care health plans, are seeking a registered nurse to provide comprehensive case management to our members in New York. Reporting to the Supervisor of Care Management, you will develop, implement, and monitor care plans in collaboration with PCPs, caregivers, and a multidisciplinary team to meet medical, social, psychological, physical, and spiritual needs. Ideal candidates hold an NYS RN license, ADN or BSN, and prefer CCM. #J-18808-Ljbffr MJHS
