Probation Department Case Managers deliver direct services to youth involved in diversion, probation, truancy, and at-risk programs. Their responsibilities include, but are not limited to, conducting screenings and assessments to determine appropriate service referrals, and monitoring compliance with court-ordered requirements in coordination with Probation Officers and supervisory staff. Case Managers facilitate both individual and group sessions addressing areas such as life skills development, emotional regulation, healthy relationships, and other topics responsive to identified risks and needs. Additionally, they collect urine specimens to support substance use disorder monitoring and ensure adherence to treatment requirements, as well as oversee compliance with mental health services, prescribed medications, and school attendance and progress. Case Managers coordinate service referrals and collect and report statistical data for grant compliance. Benefits included in position:

(This excludes Yakima County Sheriff's Office and Department of Corrections) Responsibilities: Essential Duties: Conducts youth screenings to gather information about substance use, mental health, social histories, school attendance. Exchanges information and concerns with appropriate court staff, management and service providers. Maintains youth records and documents compliance and all contacts with youth, family, providers, schools, community organizations. Runs monthly service eligibility reports for youth on diversion and probation to determine service suitability and referrals based on risk and need. Completes referral forms and forwards to community providers. Receives monthly and quarterly reports from providers outlining youth compliance with court orders. Distributes compliance reports and corresponding invoices to appropriate Probation Officers and financial unit staff. Prepares monthly statistical reports for management and administration Maintains youth case records and documents and reports compliance, UA results, provider progress, school attendance, employment status, parent/guardian contacts and other youth activity. Provides individual and group sessions to or for Juvenile Court youth such as life skills, emotional regulation, parenting, healthy relationships, truancy, and other risk and need responsive topics. Collects urinalysis, to ensure chemical abstinence, provides testimony in court regarding youth compliance and progress in skills groups. Provides youth transportation for services, educates youth and families regarding transportation, food, housing and other services in the community. Meets with youth and guardians to determine family and youth needs are being met. Promotes and advocates program services, Plans, develops and implements events and presentations, workshops, orientations and community forums. Provides program information to employers, youth, care providers, and the public regarding issues such as education, employment, substance use disorder, treatment alternatives, counseling services, and community service. Other duties required. Education & Experience: Associate degree or two-year technical certificate: Type of degree/certificate: Human Sciences or related field AND 3 years of direct service case management or social service program experience or an equivalent combination of education and experience which provides knowledge, skills and abilities sufficient to successfully perform the essential duties of the job. Required:
• Valid Washington State Driver's License and proof of insurance at time of appointment. • Successful completion of a comprehensive criminal background investigation and general employment verification. • Successful completion of a criminal and abuse/exploitation history disclosure statement pursuant to Chapter 486 Laws of 87, Sections 1 5. Criminal arrest history or history of traffic offenses may be disqualifying.
• Individual Alternative Choice training (i-ACT) and certification within one year of employment. Preferred: • Bilingual in English/Spanish • Experience working with youthful or adult offenders Equipment Used: Computer, printer, copier, telephone, cell phone, fax, projector, county passenger vehicles Working Conditions: This position is rated as Sedentary Work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
