CLINICAL INTERVIEWING - MGR FINAL DECISION Unit Information:o Beds: N/Ao Minimum Years of Experience required? 5 yearso Will you accept a first-time traveler? No Patient Types/Common Diagnoses:o Travelers will care for patients in NICU, pediatric, ER, MedSurg, Tele, Oncology, and ICU. No L&D Case Load:o As low as 15 patients and as high as 25-30, depending on staffing Required License, National Certification, Certs:o BLS, BSNo State of WA RN license Skills required:o Experience with above patientso Case management and discharge planning, strong assessment skills, ability to discharge homeless patients and discharge patients to the next level of care to SNF, IP, LTAC, Shelter, Assisted Living Facility, Medicare/Medicaid patients. Support within the Department:o Case Management Social Workerso Case Management Assistanto CM Manager dailyo Will be partnered with a CM Social Worker Technology/Equipment:o EMR: EPIC - experience preferred Floating: If yes, where?o Throughout hospital Orientation (facility and unit):o NEOo 2 weeks in department Scheduling:o Weekend rotation: Every other weekendo On call? Noo Holiday Expectations: Holidays are required according to department rotation and as needed for coverageo RTO Approval: Ok to approve 7 dayso Shift times: 0700-1730, four 10h days. 2 15 mins breaks and 1 30 min meal break.o Schedule cycle: Other notes:o Scrub Color or attire: Any solid color scrub or business casual attire

Required Qualifications

Registered Nurse, Case Manager Experience: 5 years Travel Experience: 1 assignment in Registered Nurse, Case Manager Licenses: RN(Compact Accepted) Certifications: BLS SSN Required DOB Required References: 1 Reference in the last 12 months BSN degree Previous travel experience case manager skills checklist within 12 mos 1 ref within 12 mos for job worked in last 5 yrs acute care case manger experience with following units : NICU, pediatric, ER, MedSurg, Tele, Oncology, and ICU. o Case management and discharge planning, strong assessment skills, ability to discharge homeless patients and discharge patients to the next level of care to SNF, IP, LTAC, Shelter, Assisted Living Facility, Medicare/Medicaid patients.
