Home Health Registered Nurse (RN) Case Manager Location: Springfield, Missouri and surrounding service area (within a 50-mile radius) Schedule: Monday through Friday, 8:0 AM to 4:30 PM Productivity Expectation: 25 visits per week On-Call Requirement: 3 weekdays per week and 2 weekends per month Mileage Reimbursement: Yes, between patient visits Position Overview We are seeking a compassionate and dedicated Home Health Registered Nurse (RN) Case Manager to provide exceptional hospice care to patients and families throughout the Springfield, MO service area. This role is ideal for an experienced nurse who is passionate about delivering high‑quality, patient‑centered end‑of‑life care while working collaboratively within an interdisciplinary team. As an RN Case Manager, you will coordinate and manage patient care plans, provide skilled nursing services, and serve as a key resource for patients, families, and caregivers throughout the hospice journey. Schedule & On‑Call Requirements Regular Schedule Monday through Friday 8:00 AM to 4:30 PM Weekday On‑Call 4:30 PM to 8:00 AM Three weekdays per week Weekend On‑Call Saturday 12:00 AM through Monday 8:00 AM Two weekends per month Key Responsibilities Serve as the primary RN Case Manager for hospice patients Provide comprehensive nursing assessments and ongoing patient evaluations Deliver high‑quality hospice and end‑of‑life care in accordance with physician orders and care plans Develop, implement, and update individualized care plans Coordinate care with the interdisciplinary team, including physicians, social workers, chaplains, therapists, and hospice aides Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and available support resources Maintain accurate and timely clinical documentation Participate in Interdisciplinary Group (IDG) meetings every other Wednesday Respond to patient needs during scheduled on‑call rotations Promote patient comfort, dignity, and quality of life throughout the hospice experience Required Qualifications Current Registered Nurse (RN) license Current Basic Life Support (BLS) certification Strong clinical assessment and communication skills Ability to work independently and manage a caseload effectively Valid driver's license and reliable transportation #J-18808-Ljbffr
