Job Description Job Responsibilities / Essential Functions Provides direct care to inpatients and outpatient ostomy patients with application of the Nursing Process. Performs patient assessments, implements care plan, adheres to established standards of care. Actively interfaces & assists in maintaining a collaborative relationship. Collaborates closely with physicians, APPs, fellow wound care nurses, wound program leadership, and other hospital staff to ensure service excellence. Coordinates with Manager in maintaining a safe environment of care. Adheres to OSHA, CMS, CDC, Joint Commission, NJ Department of Health, and applicable medical center standards/policies. Case Management. Identifies, coordinates, and meets patient care needs including but not limited to home care, DME orders, communication with all providers involved in the patient’s care. Serves as a clinical resource to the health care team, patients, and their families. Acts as a mentor/preceptor. Acts as an educator and consultant to staff nurses, physicians, APPs, and support staff for wound, skin and ostomy care concepts, practices, products and devices. Participates in hospital and system-wide research and educational programs and studies. Collaborates with the Manager in areas of daily operational and fiscal management. Assists in the development of wound and ostomy care policies and procedures. Able to assume super-user status when implementing new processes/equipment. Qualifications Required Bachelor of Science in Nursing. 2-5 years' experience in wound/ostomy care and general medical-surgical nursing. Wound Care Certification (e.g., WOCN, WCC, CWS), BLS for Healthcare Providers. Valid state licensure as a Registered Professional Nurse in NJ. Preferred Master's degree in nursing or related field ACLS certification.
