You will do this by integrating care from different fields, doing case reviews and consultations, and participating in the treatment planning process to ensure excellence in serves. You will help primary clinicians and community partners to connect families with services that they may qualify for to provide the best opportunities for them to succeed. This position reviews files to help assess level of care needs and provides guidance to team members on what may be beneficial next steps for high acuity cases. This position also serves as a hospital liaison to support youth who have been psychiatrically hospitalized. We are looking for someone who is detail oriented, is a productive communicator, and has good clinical skills.What We Require:Master's level licensed professional with a minimum of two years' experience in primary or behavioral health care services working with children and/or adults. You must also have one of the following Michigan Licensures: Licensed Master Social Work (LMSW) (Preferred), Licensed Professional Counselor (LPC) (Preferred), Limited Licensed Psychologist (LLP), or a Registered Nurse (RN). Limited Licenses will be considered. A valid Michigan's drivers license. A strong understanding of the needs of persons with mental illness, intellectual/developmental disabilities, and substance use disorders. May occasionally require light lifting up to 25 pounds, stooping, kneeling, crouching, or bending as appropriate. Use of computer screen for periods in excess of 30 minutes at a time. Possible eyestrain from extended periods of viewing computer screen.
