Responsibilities
- Provide direct patient care and nursing assessments for residents in an LTC/assisted living setting
- Administer medications and monitor responses, document in resident charts
- Perform routine nursing procedures, wound care, and assistance with activities of daily living
- Develop and update individualized care plans in collaboration with the interdisciplinary team
- Monitor resident safety, respond to changes in condition, and communicate with physicians and families as needed
- Ensure compliance with facility policies, infection control practices, and documentation standards

Requirements
- Experience in long-term care (LTC) or skilled nursing settings preferred
- Flexible availability to accommodate a travel assignment with 40 hours per week
- Ability to work effectively in a collaborative, interdisciplinary team
- Strong communication and documentation skills
- Comfortable operating in a dynamic, resident-centered care environment
