Make the initial evaluation and re‑evaluate the patient’s nursing needs during each visit. Communicate significant findings, problems, and changes in health conditions, environment, or unsafe facility conditions to the clinical supervisor, the physician and/or other personnel involved with patient care. Counsel, instruct, and implement the plan for patient safety, utilizing patient information, family, and community resources. Participate in the implementation of the plan of care for an assigned case load to ensure quality, continuity of care, and achievement of patient outcomes. Identify deviations from normal nursing and patient condition status and initiate appropriate actions. Verify the plan of care prior to each visit and provide care according to physician’s orders, assessment data, and established standards and guidelines. Perform other duties/projects as assigned. Required Qualifications Graduate of an accredited certificate, diploma, associate’s or bachelor’s school of nursing. At least 1 year of experience as a nurse in a clinical care setting. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. Home health experience preferred, but not required. Current state license as a registered nurse. Ability to travel approximately 50% of the time. Excellent verbal and written communication skills. You will report to the Branch Manager or Clinical Manager.
