Compassionate Hospice RN Case Manager

Gentiva is seeking an experienced Hospice RN / RN Case Manager to provide in-home nursing care and coordinate services for patients facing life-limiting illness. You will work with families to implement individualized plans of care, perform assessments, and collaborate with physicians, social workers, chaplains, and other team members to ensure compassionate, comprehensive care.
