Aftercare and Outreach Manager

POSITION SUMMARY: Responsible for assisting survivors in the RESET program and GRO clients with linkage and access to aftercare services and re-engaging previous clients where possible. Coordinate and conduct community outreach to increase awareness of human trafficking and available resources. Coordinate and provide transition and aftercare services to clients discharged from programs. JOB DUTIES AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform the following satisfactorily; other duties may be assigned. Identify available and relevant resources for youth transitioning or discharging from programs. Develop discharge and after-care plans for youth transitioning from the program. Conduct post-discharge follow-up with clients for at least 90 days. Conduct community outreach through information sessions. Meet regularly with youth in programs on aftercare planning. Facilitate aftercare groups with discharged youth. Lead presentations at area schools and other youth-centered locations. Serve as liaison to community partners to disseminate information about the program and agency. Participate in treatment team meetings to address the individualized needs of youth in the program. Collaborate with staff, youth, and community partners in creating individualized transition and discharge plans for clients. Participate in on-call rotation for the RESET program. Meet designated benchmarks and quality measures. Document/track referral sources and custom performance measures. Demonstrate a high level of integrity and confidentiality. Keep the supervisor informed of all job-related tasks, activities, and challenges. Attend administrative meetings and job-related training as directed. Other duties as assigned. EDUCATION/EXPERIENCE:

Bachelor’s Degree or higher in Social Work, Psychology, or Human Services. QMHP experience and QMHP Certification a plus One to two years of experience working with children/youth with behavioral and emotional issues. Knowledge of child growth and development

Demonstrate knowledge of computer programs and databases, including Microsoft Office (Excel, Word, PowerPoint, Outlook, and Publisher), Internet Explorer, and Extended Reach. Must have a working knowledge of the Internet and the ability to correspond via e-mail. Must have excellent time management and organizational skills. Must have excellent written and verbal communication skills. Must have excellent telephone etiquette skills. Ability to present the organization in a professional image. Ability to speak with self-confidence. Ability to produce professional correspondence and presentations. Ability to problem solve and work independently. Ability to work as a team player and work effectively with other agency employees, outside visitors, and volunteers. Conducts job responsibilities per the ethical standards of conduct, state contract, appropriate professional standards, and applicable state/federal laws. Must maintain a current driver’s license and insurance. Must be willing to transport agency residents. Must be of high moral, ethical, and professional character. Ability to spend long periods standing, bending, stooping, and walking around the facility supervising the activities and movement of staff and children. Ability to climb stairs repeatedly. Ability to lift at least 50 pounds. Mature judgment and an ability to act independently and responsibly in all matters relating to the care and safety of children. Ability to engage and effectively communicate with persons from diverse cultures and communities, as well as from different groups, including children, their parents, agency colleagues, CPS staff, support staff members, supervisors, administrators, and persons from the legal and judicial system. Skills in organizing work in a responsible way within an environment with multiple challenges, priorities, and time frames.

CERTIFICATION/LICENSES/TRAINING

Obtain 40 hours of CEUs – Annually Emergency Behavior Intervention Training/TBRI – Bi-Annually.
