Job Description **What You'll Be Doing** Ready to be the MVP of patient care transitions? Here's where the magic happens:
- *Be the First Responder:** Catch new referrals coming in via phone, fax, and computer—you're the gatekeeper to great care!
- *Data Entry Ninja:** Enter patient demographics and referral information into our system with speed and accuracy. Think of it as building the foundation for excellent care.
- *Insurance Detective:** Work with our Billing department to crack the code on insurance coverage and get patients the support they need.
- *Care Coordination Guru:** Help orchestrate smooth patient care transitions and keep the home care journey running seamlessly.
- *Communication Champion:** Connect with physicians, nurses, case managers, and the public—you're the bridge that makes everything work together.
