Work Environment

This position is an onsite role where you will provide rehabilitation services in the comfort of patients' homes. Working in home care allows for a unique therapeutic atmosphere where you can witness patients in their natural environments, facilitating more personalized and effective treatment strategies. You will primarily engage with clients one-on-one, ensuring that each therapy session is tailored to individual needs in a safe and comfortable space.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions to establish a baseline for treatment.
• Develop and implement personalized treatment strategies focused on improving physical function, enhancing mobility, and reducing pain.
• Utilize modalities and advanced therapeutic techniques to provide effective care, including therapeutic exercises, manual therapy, and patient education.
• Maintain meticulous documentation of patient assessments, treatment plans, and progress notes in accordance with clinic protocols and insurance requirements.
• Collaborate closely with multidisciplinary healthcare teams to integrate care approaches and optimize patient outcomes.
• Ensure compliance with clinical productivity standards while actively seeking to enhance personal and team performance through ongoing education and training.
• Participate in regular team meetings to contribute to program development and share best practices.
• Help safeguard a safe environment for patients by adhering to all health and safety protocols and maintaining therapy equipment.

Patient Population / Client Focus

The ideal candidate will work extensively with a diversely aged patient population, including but not limited to individuals recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. Our focus is on providing empathetic care that caters to the specific rehabilitation needs of each client while fostering their independence through skilled therapeutic interventions.

Qualifications

• A Master's or Doctorate degree in Physical Therapy from an accredited institution.
• An active Physical Therapy license in Florida is required; applicants may also be considered if they are eligible for licensure.
• Demonstrated passion for providing high-quality patient care and a collaborative approach to achieving the best patient outcomes.
• Proficiency with electronic medical records (EMR) software, with familiarity with WebPT being a plus.

Preferred Experience

• A minimum of 2 years of clinical experience working as a Physical Therapist, specifically in home care settings is preferred.
• Experience in orthopedic rehabilitation is highly desirable.
• Background in patient education and training methods.

Compensation and Benefits
