Lifeworks is hiring a full time Case Manager to work with Individuals with ID/DD. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We’re searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve. Responsibilities Provide individual and small group instruction in self‑care, activities of daily living, communication, social and motor skills. Provide direct care for ADLs and group activities. Lift and transfer individuals as appropriate to their needs. Coordinate services with Interdisciplinary Team. Communicate respectfully and pro‑actively with families, residential staff and all members of participants’ teams. Develop and implement Day Habilitation Service Plans (DHSP), including: Maintain progress notes and document progress on objectives. Maintain assigned participant records so that all information is current and filed appropriately. Write bi‑annual and annual summaries. Complete all required documentation in a timely and accurate manner. Participate in scheduled supervision sessions with Assistant Director. Participate in program‑wide staff meetings and case review meetings as scheduled. Participate in team meetings regarding general program development, including curriculum development and schedules. Ability to demonstrate good computer skills as well as ability to learn to utilize the agency’s computer programs for documentation of necessary data. Attend in‑service trainings as required. Ability to work in a team environment. Ability to multitask and prioritize tasks. Strong written and verbal communication skills along with excellent interpersonal skills. Comply with all applicable State and Federal regulations, agency policies and procedures, and personnel practices. Protect the health and safety of all participants and staff. Performs other duties as requested by supervisor(s). Requirements Experience working with Individuals with ID/DD A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services. Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks. Valid Driver’s License required. Valid MAP certification is preferred.
