Responsibilities
- Build and maintain strong relationships with hospital case managers, discharge planners, social workers, physicians, and other referral partners.
- Conduct daily hospital rounds and referral source visits throughout the Birmingham market.
- Educate healthcare professionals on Champion Care's skilled nursing, short-term rehabilitation, long-term care, respite care, and post-acute services.
- Review referrals and coordinate admissions with facility leadership and admissions teams.
- Develop strategies to increase referrals, admissions, and overall census growth.
- Identify new business development opportunities and cultivate referral partnerships.
- Track referral activity, admissions, and market trends to achieve performance goals.
- Represent Champion Care at networking events, healthcare meetings, and community outreach activities.
- Serve as a knowledgeable resource regarding patient placement, insurance considerations, and post-acute care options.
- Maintain strong communication with facility leadership to ensure exceptional customer service and referral satisfaction.
- Previous experience in healthcare sales, hospital liaison, physician relations, admissions, business development, case management, discharge planning, or healthcare marketing preferred.
- Skilled nursing facility (SNF), rehabilitation, long-term care, hospital, home health, hospice, or post-acute care experience strongly preferred.
- Proven ability to build and maintain professional relationships.
- Excellent communication, presentation, negotiation, and organizational skills.
- Self-motivated with the ability to work independently and achieve goals.
- Valid driver's license with reliable transportation.
- Ability to travel throughout the Birmingham market.
