Job Summary We are seeking a skilled Home Health Physical Therapist (PT) – PRN – Per Visit – Home Health to join our team in Dallas, TX. The ideal candidate will have experience in providing physical therapy services to patients in their homes. As a Home Health Physical Therapist (PT), you will be responsible for assessing patients’ needs, developing treatment plans, and providing therapy services to help patients regain their mobility and independence. Location North Dallas, TX Responsibilities Conduct initial assessments of patients’ physical abilities and develop treatment plans Provide therapy services to patients in their homes Monitor patients' progress and adjust treatment plans as needed Communicate with patients’ physicians and other healthcare professionals to coordinate care Document patients’ progress and maintain accurate records Provide education to patients and their families on how to manage their conditions and prevent further injury Requirements Current license to practice as a Physical Therapist in the state of Texas Minimum of 1 year of experience in home health Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong problem‑solving and critical thinking skills Valid driver's license and reliable transportation #J-18808-Ljbffr Frontpointhealthcare
