Early Intervention Service Coordinator

Purpose of Position- Works with families of children ages 0-3 to access and navigate early intervention services by providing service coordination to an assigned case load. Develops and implements an Individual Family Services Plan. Coordinates and monitors the implementation of services/supports written in the IFSP. Assists families in acquiring services and supports. Maintains regulatory compliance, including ensuring documentation is completed to obtain and maintain ongoing funding for services for children and families on their case load. Coordinates delivery of services and supports. Advises families on appropriate service options and assists in acquiring needed services/supports. Coordinates the delivery of services and supports from A&I Avenues and other organizations providing services/supports to the child and/or their family. Utilizes professional discretion in consulting with providers to ensure best practices in service delivery. Assists families in resolving complaints with service providers. This may include the formal appeal process. Facilitates exits for children and families, as needed or requested, including ensuring that proper notice of discharge is provided. Provides support and resources to families as needed. Mediates for, and negotiates with, IFSP team members and service entities to ensure appropriate and effective services delivery for clients. Contacts families monthly and documents the contact in electronic health record. Completes Targeted Case Management billing monthly. Completion of timely, accurate, and professional case notes within established timelines. Ensures that all required documentation is in accordance with applicable Federal, State, and A&I Avenues rules and regulations. Ensures that a master file for each child is maintained in accordance with applicable rules and regulations. Ensures the completion of paperwork required to obtain and maintain availability of funding for services/supports for each child on the case load. Responsible for the development and implementation of the IFSP. Leads the IFSP team in both periodic and annual reviews in the maintenance and development of the IFSP for client. Ensures written document is completed and distributed. Facilitates communication throughout the year regarding the IFSP and related services/supports. Monitors services through the year to ensure alignment with the current client needs and the IFSP. Ensures that the IFSP team reviews and revises the IFSP on a six month and yearly basis, or more often as needed by the family or team. Other Duties/Responsibilities Other duties as assigned. If position occupant has a Master’s of Social Work, may supervise or assist in supervision of social work interns placed at A&I Avenues. Advocates for the development of services/supports needed, but not available, for people with developmental disabilities and/or their families. Job Qualifications Knowledge, Skill, and Ability: Solid skills in working with people with developmental disabilities/delays and/or their families. Solid independent judgement and discretion in monitoring, advising, and advocating for individuals and their families. Solid knowledge of family dynamics. Ability to work independently and prioritize tasks/goals for self and others. Effective written and verbal communication skills with individuals and groups at all professional levels. Strong planning and organizational skills. Effective and creative negotiator and problem solver. This includes an ability to provide crisis intervention as well as mediate disputes. Ability to work effectively with various technology, including word processing software, internet based databases, etc. Possession of a valid driver’s license and ability to meet A&I Avenues’ driving requirements. This position requires regular use of a personal mobile device, such as a smartphone or tablet. Education: Bachelor’s degree in human services-related field is highly preferred. OR Five years’ work/volunteer experience with long-term care and/or disabilities programs and services. OR Some combination of relevant education and experience appropriate to the requirements of the position. Working Environment/Physical Activities Effective with shifting roles, responsibilities, and expectations in a highly stressful and changing work environment. Office and remote hybrid work. Ability to work autonomously while remote. Ability to respond to emergency situations, as needed.
