Job Summary:

Responsibilities:

• 
  Deliver direct nursing care to patients in accordance with physician orders and facility protocols.
  
  
• 
  Assess patient conditions and document clinical findings accurately.
  
  
• 
  Collaborate with interdisciplinary team to develop and implement care plans.
  
  
• 
  Administer medications and treatments following safety guidelines.
  
  
• 
  Educate patients and families on health management and post-discharge care.
  
  
• 
  Ensure compliance with healthcare regulations and facility policies.
  
  

Qualifications:

• 
  Valid Registered Nurse license in the state.
  
  
• 
  Minimum of 1-year experience in LTC/SNF settings.
  
  
• 
  Demonstrate excellent clinical assessment and documentation skills.
  
  
• 
  Possess current BLS certification.
  
  
• 
  Ability to adhere to infection control protocols and safety standards.
  
  
• 
  Strong communication and teamwork abilities.
