Job Summary:

Responsibilities:

  Administer medications and perform treatments accurately as prescribed.

  Monitor patient vital signs and assess health status regularly.

  Document all clinical interventions and patient responses promptly.

  Collaborate with healthcare team to develop and update care plans.
  
  
• 
  Maintain compliance with facility policies, safety standards, and infection control protocols.
  
  

Qualifications:

• 
  Active LVN or LPN license required.
  
  
• 
  Proven experience in LTC or SNF environments preferred.
  
  
• 
  Demonstrate excellent communication and observational skills.
  
  
• 
  Commit to maintaining a high standard of patient care and safety.
