What You'll Do:

• Provide high-quality primary care to both pediatric and adult patients, managing a varied panel with a focus on chronic pain and preventive health.
• Ensure efficient chart closures within 72 hours to maintain seamless patient records.
• Collaborate closely with a multidisciplinary team to deliver personalized treatment plans.
• Maintain compliance with all credentialing and immunization requirements prior to start date, including flu vaccines, TB testing, and titers for MMR, Hep B, Varicella, and Tdap.

Required Skills:

• Minimum of 3 years of experience in a similar outpatient setting or panel management role.
• Strong clinical judgment and excellent patient communication skills.
• Ability to manage a busy patient load, averaging 18 patients per day, Monday through Friday, 8 am to 5 pm.
• Experience in chronic pain management is highly preferred.
• Active DEA license before start date.
• Proficiency in electronic medical records and documentation.

Nice to Have Skills:

• Additional certifications in pain management or specialized adult/ pediatric care.
• Bilingual abilities to serve our diverse community.
• Experience with telehealth or remote patient monitoring.

Preferred Education and Experience:

• A Master's Degree in Nursing or Physician Assistant Studies from an accredited program.
• Current state licensure as an NP or PA.

Other Requirements:

• Must have completed all required immunizations and obtained necessary titers before onboarding.
• Credentialing process takes approximately 45-60 days.
• Efficient time management and dedication to providing excellent patient care.
