Job Details Position: Per Diem Physical Therapist Assistant (PTA) – Home HealthLocation: Arlington Heights, IL 60005 Position Type: Per DiemEducation Level: Physical Therapist AssistantSalary Range: $50.00 - $60.00 per Hour (per visit) Center of Illinois Home Healthcare, Inc. is a Medicare‑certified, ACHC‑accredited home health agency seeking a compassionate and dependable PTA to join our interdisciplinary team. The role offers a flexible schedule and provides high‑quality physical therapy services under the supervision of a licensed Physical Therapist in patients' homes. Responsibilities Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist. Follow the established plan of care and treatment goals developed by the supervising Physical Therapist. Observe, document, and report patient responses to treatment and communicate changes in condition to the supervising Physical Therapist. Educate patients and caregivers on prescribed exercises, mobility techniques, fall prevention, and home safety. Complete accurate and timely clinical documentation in accordance with Medicare, CMS, and agency guidelines. Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary care team. Participate in care coordination and case conferences as needed. Maintain compliance with agency policies, state regulations, and accreditation standards. Qualifications Current Illinois Physical Therapist Assistant (PTA) license required. Minimum of one year of home health experience preferred. Knowledge of Medicare home health regulations and documentation preferred. Experience with Axxess EMR is a plus. Strong clinical, communication, and organizational skills. Current CPR certification. Valid driver's license, reliable transportation, and current auto insurance.
