Job Details Job Location: Arlington Heights, IL 60005 Position Type: Per Diem Education Level: Physical Therapist Salary Range: $75.00 - $100.00 Hourly Travel Percentage: Up to 25% Per Diem Physical Therapist – Home Health Center of Illinois Home Healthcare, Inc. is seeking a compassionate and motivated Per Diem Physical Therapist to join our Medicare-certified, ACHC-accredited home health agency. This position offers the flexibility to create your own schedule while providing high-quality physical therapy services to patients in their homes. As a member of our interdisciplinary team, you will evaluate, develop, and implement individualized treatment plans that promote independence, improve mobility, and enhance patients' quality of life. Responsibilities Evaluate patients and develop individualized physical therapy treatment plans. Provide skilled physical therapy services in patients' homes according to physician orders and the plan of care. Assess patient progress and modify treatment plans as clinically appropriate. Educate patients, families, and caregivers on therapeutic exercises, mobility, fall prevention, and home safety. Complete timely and accurate documentation in accordance with Medicare, CMS, and agency guidelines. Collaborate with physicians, nurses, occupational therapists, speech therapists, home health aides, and other members of the interdisciplinary team. Participate in care coordination and case conferences as needed. Maintain compliance with agency policies, state regulations, and accreditation standards. Qualifications Current Illinois Physical Therapist license required. Minimum of one year of home health experience preferred. Experience with Medicare home health regulations and documentation preferred. OASIS experience preferred. Experience with Axxess EMR is a plus. Excellent communication, clinical assessment, and organizational skills. Current CPR certification. Valid driver's license, reliable transportation, and current auto insurance.
