Bilingual Subsidy Wraparound Case Manager Position: Bilingual Subsidy Wraparound Case Manager
Pay Rate: $22.73-$27.25 Status: Non-Exempt Last Revised: 06/2026 Definition: Wraparound case management services are available to every child and family receiving support from The Children's Cabinet. Our approach centers on family strengths, community supports, and sustainable, long-term solutions, all facilitated through team-based decision-making processes. These services encompass child and family advocacy, crisis intervention, basic needs security, general case management, and streamlined access to community resources through closed-loop referrals. Wraparound Case Managers collaborate with local public service agencies and other nonprofits to address the full spectrum of family needs. Our wraparound case management model prioritizes four key elements: 1) Family/Client-directed, 2) Driven by underlying needs, 3) Rooted in an evidence and strengths-based approach, and 4) Supported by an effective team process.
The Bilingual Subsidy Wraparound Case Manager is a specialist level position who is responsible for assisting clients with intense referral needs in addition to subsidy eligibility. The Subsidy Wraparound Case Manager will assist clients with any eligibility requirements, referrals to outside agencies, and will follow-up to ensure the client's needs were met. Supervision: This position has no direct supervisory responsibilities. Key Responsibilities:
- Develop a thorough understanding of the agency's mission, strategic focus areas, and organizational objectives, actively contributing to their success. - Read, understand, and consistently apply all agency and department policies and procedures, and adhere to ethical standards as well as applicable regulations. - Maintain a comprehensive knowledge of agency programs and community resources to provide accurate information and referrals to families during case management. - Participate in program-related activities, including child and family team meetings, to stay engaged and informed. - Attend The Children's Cabinet-sponsored events and activities as requested (e.g., ART of Childhood, field trips, graduations, and awards ceremonies). - Conduct intake assessments to evaluate client and family needs, strengths, priorities and goals. - Develop individualized, family-centered service plans with clear and measurable objectives, monitoring progress and adjusting as needs evolve.
- Attend community outreach events and follow-up with participants afterwards to ensure connection to services.
- Accept and return phone calls from families requesting childcare services provide program guidelines, application process and determine initial eligibility for programs. - Enroll eligible families by obtaining necessary documentation and informing parents of their rights and responsibilities.
- Obtaining updates on employment, training and income; re-certifying families as required by program guidelines; and making referrals to additional community resources and following up with families to ensure they receive the additional services needed. - Process termination of coverage for families no longer eligible for services, including notification of providers. - Collect and maintain statistics necessary for program compliance, enrollment options and reports.
- Assist parents with complaints about program and/or childcare services, mediate as needed, and advise of proper procedure for resolution of grievances. - Assist with Child Care Resource and Referral process as needed. - Perform travel related duties as needed to ensure adequate administration of the program. - Provide wraparound case management services to families and youth involved in Cabinet programs, ensuring tailored support based on individual and family needs. - Conduct home visits and field activities to evaluate clients' needs within their own environments, ensuring personalized case management support is delivered when and where it is most needed. - Serve as an advocate for clients, assisting them in accessing essential community resources and services. - Guide clients through various systems, including healthcare, social services, housing, and education. - Empower clients to become self-sufficient by encouraging self-advocacy and the use of available resources. - Respond to client crises by providing immediate assistance, emotional support, de-escalation and solutions. - Connect clients with emergency resources, such as shelters, food assistance, and mental health services. - Identify and connect clients with appropriate resources for basic needs, mental health, employment, and education. - Maintain a network of current community resources and referral options, educating clients about available services and supporting them throughout the referral process. - Actively engage in community events and outreach activities that support The Children's Cabinet's mission and help connect clients with additional resources. - Accurately document client interactions, service plans, and progress in case management databases. - Utilize case management software to track data, monitor outcomes, and ensure all information is kept confidential and compliant with data privacy standards. - Prepare and submit monthly reports as directed to support program evaluation and accountability. - Prepare case summaries and participate in team-based case reviews to assess client progress and adjust service plans as necessary. - Assess eligibility for youth participating in various programs and, when needed, provide transportation support for families and youth to attend required appointments or activities. - Participate in in-service training and other staff development opportunities to maintain and enhance professional skills. - Provide regular updates to supervisors regarding case status, challenges and successes. - Perform additional duties as assigned to support the agency's mission and contribute to the overall success of its programs. Minimum Qualifications Needed Knowledge of: - Appropriate interactions with children and families - Community agencies and local resources, including an understanding of the challenges facing youth and families - Positive parenting practices and work/life support for families - Family and youth relationship issues - Sound decision-making and crisis management techniques - Case Management skills with an emphasis on wraparound services and care coordination - Basic Computer applications (e.g., Word, Excel, Outlook, SharePoint, etc.) Ability to: - Be self-motivated, team player and willingness to learn new systems and software - Keep appointments punctually, organized and demonstrate follow through - Communicate ideas effectively, clearly and concisely both orally and in writing - Adhere to schedules and specific timelines - Work with families of diverse backgrounds and beliefs and adapt service delivery accordingly - Problem-solve in stressful situations - Use excellent inter-personal skills, including intangible qualities (e.g., friendliness, openness, non-judgmental attitude, enthusiasm, empathy etc.) - Collaborate with staff, school district personnel, and community agencies - Perform all essential job functions including lifting up to 40lbs, navigating stairs and sitting at a computer for extended periods of time. - Serve as a role model (positive attitude, professional dress, demeanor, and behavior - Work as a member of a multidisciplinary team - Adapt Welfare regulation changes timely and accurately Special Requirements - Able to pass complete background check. - Bachelor's degree preferred in Related Field or equivalent combination of education and experience in early childhood through adolescence. - At least three years of experience in community service or case management position - Bilingual (English/Spanish) required
- Must have reliable vehicle for work and maintain a clean driving record
- Must possess a valid Nevada Driver's License, proof of current automobile insurance for utilizing a personal vehicle for company business, and a clean driving record. - Position may require evening and weekend hours based on program and family needs. - Work requires some weekend and evening hours. - Requires some travel to Offices outside of the Reno Sparks area and may include training within and outside of the state.
