Community Health Worker: Home Visits, DME & Telehealth
Your Health Organization
Your Health Organization in Georgia is seeking a dedicated Community Health Worker to serve patients in the Athens area. The CHW will travel to homes and ALFs to coordinate care, deliver medications and DME, and facilitate visits with providers. A valid driver’s license and reliable transportation are required for daily travel. The role is full-time, Monday to Friday, 8:00 AM–5:00 PM, with a focus on ensuring access to care and improving patient outcomes in the community. #J-18808-Ljbffr Your Health Organization
