Community Health Advocate: Home Visits & Telehealth
Your Health Organization
Your Health Organization is seeking a dedicated Community Health Worker to serve patients in Blue Ridge, GA and surrounding areas. The CHW will visit homes and ALFs, coordinate with providers, deliver medications and DME, and support RPM and telehealth activities to improve access to care. The role emphasizes patient-centered care, collaboration with care teams, and ensuring timely, safe access to essential supplies and services.
