Key Responsibilities\n

• Conduct ongoing assessments to evaluate mental health needs and tailor treatment plans accordingly.

• Communicate effectively with supervisors, healthcare teams, and family members to provide comprehensive updates on client progress.

• Develop schedules for individual therapy sessions, including home visits and observations, ensuring thorough monitoring of client progress.

• Coordinate home visits and document in-home observations, chart reviews, and data analyses to enhance care delivery.
\n
• Ensure that program plans are executed effectively and adhere to organizational standards for best practices.
\n
• Actively participate in Interdisciplinary Team Meetings to foster collaboration and align treatment strategies across disciplines.
\n
• Complete comprehensive monthly and quarterly progress notes that track client advancements and inform future interventions.
\n
• Provide training and guidance to team members on implementing program plans and accurate documentation practices.
\n
• Offer follow-up interviews and observations as needed to continually support and adjust care plans in response to client progress.
