Key Responsibilities\n

• Conduct comprehensive assessments of patients to tailor therapeutic plans that effectively address their individual needs and goals.

• Maintain clear and consistent communication with supervisors, family members, and other professionals involved in the patient's care to ensure comprehensive support.

• Develop a structured schedule of appointments, home visits, and assessments to facilitate patient progress monitoring and support continuity of care.

• Lead and coordinate home visits as required, ensuring thorough evaluations through observations, chart reviews, staff interviews, and data analysis.

• Monitor the implementation of program plans to ensure their effectiveness in meeting the patients' therapeutic needs.

• Engage actively in interdisciplinary team meetings to share insights, provide updates, and collaborate on strategies for enhancements in patient treatment.

• Document monthly and quarterly progress notes with accuracy to track patient advancements and inform ongoing treatment decisions.

• Organize and provide training sessions to certify staff in the execution of program plans and documentation procedures.

• Offer follow-up support through extended outreach, including additional interviews and in-home visits to ensure continued patient engagement and success.

Patient Population / Client Focus\n

The Advocate Aurora Therapeutic Center primarily serves adults and youth experiencing various mental health challenges, including but not limited to anxiety disorders, depression, substance use issues, and trauma-related conditions. Your role will involve employing evidence-based therapeutic practices and culturally competent approaches to effectively address their diverse needs.

Qualifications\n

• A Master's or Doctorate degree in a relevant mental health field is required.

• Active and unrestricted licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in the state of California.

• Minimum of 2 years of counseling experience, specifically within a clinical environment.

• A strong understanding of legal and ethical standards related to mental health practice.

• Proven ability to function collaboratively within a multidisciplinary healthcare team.

Compensation and Benefits\n
