Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for elderly clients, evaluating their emotional, mental, and social needs.

• Develop individualized care plans that are tailored to the specific needs and preferences of each client and their families.

• Provide counseling and emotional support throughout the caregiving process, helping families navigate challenges such as grief, loss, and transitions in care.

• Advocate for the rights and needs of clients, facilitating access to necessary community resources and support services.

• Collaborate with an interdisciplinary team on case management to ensure a seamless and coordinated approach to the client’s care.

• Visit clients regularly via telehealth or home visits to monitor their well-being, assess living conditions, and address safety concerns.

Qualifications\n
