Hospice RN Case Manager

Job Title Part-Time RN Case Manager Company Athena Hospice of Rhode Island Job Summary Athena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our dedicated team! As a Hospice RN Case Manager, you will be responsible for planning, organizing, and directing the hospice care plan for assigned patients under the direction of the clinical director. We offer a competitive hourly rate of $45 to $50, with weekly pay, and travel throughout Rhode Island is required. Responsibilities Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs Provide a complete physical assessment and history of current and previous illnesses Supervise hospice aides Provide professional nursing care by utilizing all elements of the nursing process Assess and evaluate patient status, writing and initiating a plan of care, and regularly re-evaluating patient and family/caregiver needs Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions Provide counseling to patients and families in meeting nursing and related needs Requirements Active and unencumbered RN license in Rhode Island Nursing degree from an accredited college, university, or school Prior experience in hospice is preferred Available for Eligible Team Members Employer-Paid Life Insurance 401(k) with Employer Match Vacation and Personal Time Health, Dental, and Vision Insurance We are an equal opportunity employer that values diversity at all levels.
