Roles & Responsibilities:

• Deliver coordinated nursing care for assigned patients according to established standards of care.
• Supervise and direct activities of nursing staff and coordinate care with other healthcare disciplines.
• Collect and analyze relevant patient data to determine care issues, developing appropriate care plans.
• Implement care plans, coordinating care delivery to promote health and maintain a safe environment.
• Evaluate progress towards patient outcomes and make necessary adjustments.
• Collaborate with patients, families, and healthcare teams to provide comprehensive care.
• Provide educational opportunities for patients and family members.
• Participate in discharge planning to ensure continuity of care.
