Bilingual-Case Manager, Transitional Foster Care

Position Summary As an Unaccompanied Children (UC) Transitional Foster Care (TFC) Case Manager, this position is primarily responsible for providing casework and supportive services to children in foster care, foster families, and biological families referred to refugee and immigrant services TFC program. This position must provide child-facing services on-site. This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements. Annual Salary Range: $44,200.00 - $52,000.00 Essentials Duties and Responsibilities Coordinates case management services for children in care under the direction of the Lead Clinician/Lead Case Manager to include, coordinating timely referrals to external care providers and consistent communication with relevant stakeholders on referral status, ensuring consistent care coordination communication between medical and behavioral health staff internally and externally, coordinating timely referrals to child advocate services as needed, ensuring the appropriate health interpretation and translation services are available, and ensuring the coordination of the appropriate post‑release services that include the continuity of healthcare services. Completes initial intake assessment within 24 hours of arrival and ensures youth feel safe and well oriented to the agency and foster home. Promptly coordinates contact with youth sponsors immediately upon arrival, while also ensuring the sponsor obtains all applicable documentation such as the Family Reunification program (FRP) packet. Identifies a sponsor by working with the consulate or equivalent resources to assign a sponsor for the youth, if unknown upon youth's arrival. Effectively utilizes a trauma‑focused lens to complete the FRP in a timely manner to ensure youth’s safe release. Ensures that all assigned youth receive mandatory services while in care to include but not limited to mental health, medical, recreational, educational, and other supportive services in accordance with the time expectations of the program. Maintains accurate and current information on foster children in care, while ensuring Office of Refugee Reinstatement (ORR) and Catholic Charities West Michigan (CCWM's) internal documentation practices and reporting requirements are met. Attends weekly meetings with supervisor to consult on cases, to review case plan and direction, and to evaluate client participation and progress. Assists the licensing team to ensure foster families are well trained and equipped to support the UCs in their home. Schedules meetings with foster parents and foster children in the foster home on at least a monthly basis and address any concerns in service delivery. Translates both written and verbal communication for program clientele and staff, if bi‑lingual. Maintains effective communication with community agencies or individuals involved in service provision. Attends internal and external training and apply newly gained knowledge in providing services. Attends and prepares for required court hearings, if applicable. Transports clients on an as needed basis. Provides child‑facing services on‑site. Drives for Agency business. Performs other special assignments at supervisor’s request. Work Environment This position is primarily office based, however, on occasion it may require travel to other agency as required by position responsibilities and/or agency management. The position is on‑site — must be willing to work entirely in person, however, there may be times that ad hoc or situational telework is needed. This position will be routinely exposed to typical office noise levels and/or exposure to various weather conditions. Ability to provide excellent and compassionate customer service. Ability to work independently and exercise good judgement, and a high level of confidentiality. Excellent verbal and written communication skills. Demonstrated clinical, therapeutic, and crisis intervention skills. Mature and stable judgement as well as sensitivity to various cultures and the unique history of refugees. Knowledge of the National Association of Social Work Code of Ethics and the NASW Standards for the practice of direct social work. Computer and typing skills sufficient to perform essential job functions. Ability to relate to diverse populations and cultures while communicating with clients and the public in a courteous and effective manner. This position requires reliable transportation. Must have a valid driver’s license and good driving record with zero work restrictions. Must have personal auto insurance as required by law with minimum bodily injury coverage limits of $250,000 per person/$500,000 per incident or $500,000 combined. Must be willing to work irregular hours including evenings, holidays, and weekends. This is an on‑site role that requires regular, full‑time attendance at the office; remote or hybrid work is not available, however, there may be times ad hoc or situational telework is needed. Successfully pass an annual TB test and document preference to receive or decline Hepatitis immunization. Must show documentation proof of immunity to vaccine‑preventable diseases transmitted by the respiratory route. Drives for agency business. Must submit to agency approved background checks. Recommended Employment Qualifications Education: A bachelor's degree in behavioral sciences, human services or social services fields is required. Experience: A minimum of 1 year of prior employment experience in child and family services or counseling is strongly encouraged. A minimum of 1 year of prior employment experience working with children and families from diverse cultures with sensitivity to cultural difference and norms is strongly preferred. Bilingual Spanish/English fluency is highly preferred. Professional Certificates, Licenses, and Registrations: Valid CPR and First Aid certified upon hire or within 60 days of hire is required. Supervisory Responsibilities: This position does not have supervisory responsibilities.
