Quality Improvement Specialist - Elevate Patient Care & Outcomes
Callen-Lorde Community Health Center
Callen-Lorde Community Health Center seeks a Quality Improvement Associate to support the Quality team in advancing clinical performance and patient care outcomes. Reporting to the Manager of Quality Improvement, you will monitor key quality measures, identify care gaps, and coordinate outreach to ensure preventive services and chronic disease management are delivered. You will collaborate with providers, care teams, and external partners to drive data-informed improvements and strengthen #J-18808-Ljbffr Callen-Lorde Community Health Center
