You will be responsible for providing essential counseling services that support patients on their journey to recovery, working collaboratively with both clients and multidisciplinary teams to ensure comprehensive care.

This position is fully remote, allowing professionals to engage with patients from the comfort of their own space while maintaining a flexible work-life balance. As a member of our team, you will collaborate closely with the Clinical Director and Executive Director to create a nurturing and therapeutic environment for our clients.

Key Responsibilities\n

• Conduct thorough assessments of client counseling needs, utilizing evidence-based practices.

• Evaluate client substance use disorders through established criteria such as DSM for diagnosis and ASAM for determining the appropriate level of care.
\n
• Develop and document comprehensive treatment plans that address individual substance abuse issues and promote long-term recovery.
\n
• Formulate appropriate discharge and continuum of care plans, ensuring clients transition effectively to different levels of care as needed.
\n
• Maintain accurate and up-to-date clinical documentation, including counseling records, assessments, and follow-up notes.
\n
• Manage a caseload of clients, performing case management and utilization review updates as necessary.
\n
• Coordinate support services for clients and their families, ensuring holistic care is provided.
\n
• Engage with referral sources such as legal entities, schools, or employers to provide comprehensive support for clients.
\n
• Utilize a multidisciplinary team approach to address crisis situations and facilitate effective interventions.

Patient Population / Client Focus\n

You will primarily focus on clients facing substance use disorders, many of whom may also experience co-occurring mental health issues. Our clients range from young adults to older individuals, each presenting unique challenges and barriers to recovery. Your expertise will help them navigate their treatment journey, fostering a supportive environment that empowers their growth and resilience.

Qualifications\n

• A Bachelor’s degree in Social Work or related field is required; an LCSW designation is necessary.

• Demonstrated experience in addiction counseling or working with individuals who have co-occurring disorders is preferred.

• Strong proficiency in assessing behavioral health needs and creating individualized treatment plans.

• Exceptional listening and communication skills, with a strong emphasis on building rapport with clients.

• An understanding of clinical documentation practices and standards of care in social work.

• Ability to work independently while also collaborating effectively with a remote team.

Preferred Experience\n
