Housing Stability Case Manager II

Catholic Charities SF HOME provides homeless and low-income families with monthly rental subsidies and intensive services to help them transition to housing and stability. The SF HOME Case Manager will manage a caseload of 18-22 families, coordinate housing searches, crisis intervention and referrals, and conduct home and field visits. Responsibilities include maintaining client records, data entry in CARES, One System and Launch Pad, monthly contacts, and collaboration with landlords and #J-18808-Ljbffr Catholic Charities
