Care Services Coordinator Responsible for assisting patients, their families, and providers across all aspects of their health care continuum. Acts as liaison between patient, medical providers, and psychiatric consultants to meet individualized needs and improve quality of life through communication, education, and coordination. Core Responsibilities Support and coordinate mental and physical health care of patients in conjunction with their medical providers and, when appropriate, other mental health providers. Assist the department with Care Services Coordination for patients under the direction of medical providers and conduct administrative functions under Department Management. Screen and assess patients for common mental health and substance abuse disorders using Mira Health Care Management software. Provide brief behavioral interventions using evidence‑based techniques such as behavioral activation, problem‑solving treatment, motivational interviewing, or other modalities as directed by medical providers. Facilitate patient engagement and follow‑up care, promoting timely access to all aspects of care and adherence to care plans developed with the patient, primary care provider, and family. Track, document, and monitor patients for changes in clinical symptoms, treatment response, and potential side effects or complications via the EHR, including in‑person or telephone encounters. Participate in regularly scheduled consultations with the psychiatric consultant to communicate treatment recommendations to the patient’s medical provider. Conduct case reviews (telephone, video, or in‑person) for patients not improving as expected; facilitate treatment plan changes in collaboration with the medical provider and psychiatric consultant. Develop relapse‑prevention self‑management plans with patients who have achieved treatment goals and are preparing for discharge. Assist patients in navigating the healthcare continuum and schedule appointments to ensure follow‑up with primary care providers and specialists. Facilitate referrals for clinically indicated services outside the organization (e.g., social services, vocational rehabilitation, substance abuse treatment), ensuring proper authorization and coordination. Obtain lab results, diagnostic studies, and consult notes, and prepare patients for upcoming appointments. Use risk stratification tools to identify gaps in treatment and track patient follow‑up and clinical outcomes. Communicate patient progress and treatment recommendations in the EHR and other required systems for sharing with providers and the psychiatric consultant. Promote educational outreach about common mental health and substance abuse disorders, empowering patients to engage in self‑management and shared decision‑making. Support medication management as prescribed by medical providers, focusing on adherence monitoring, side effects, and treatment effectiveness. Collaborate with a diverse range of people and establish effective working relationships with all team members. Follow up with patients to ensure understanding of agreed upon goals and completion of scheduled appointments and education classes. Perform other duties as assigned. Required Qualifications High school diploma or equivalent. Bachelor’s degree in a related field (equivalent years of experience may substitute for a degree). Minimum of 1 year of experience in a related field within a healthcare setting. Strong organization and time‑management skills. Excellent oral and written communication skills. Comfortable with computers and medical terminology. Knowledge of HIPAA compliance and various health insurance types. Preferred Qualifications Basic Life Support (BLS) certification from the American Heart Association. Required Licenses / Certifications Fire Life Safety Training (LA City); if no card upon hire, must obtain within 30 days and renew before expiration.
