Case Manager

Next Point HR partners with CEOs, founders, and executive teams to align people strategies with business objectives. We have been retained to identify a Case Manager for a nonprofit organization based in Panorama City, CA, dedicated to serving older adults through the Senior Community Service Employment Program (SCSEP). SCSEP is a federally funded program designed to support individuals aged 55 and older who are seeking to reenter the workforce. The program provides paid, community-based training opportunities that help participants build skills, gain confidence, and transition into unsubsidized employment. This organization is deeply committed to empowering seniors by fostering economic independence, dignity, and meaningful engagement within the community. Role Description We are seeking a Case Manager who will be responsible for supporting program participants by coordinating services, maintaining participant records, and ensuring program activities are delivered effectively and in compliance with organizational policies and procedures. This role involves participant recruitment, eligibility verification, case management, community partnership development, and program administration. The Case Manager works closely with participants, partner organizations, and internal staff to help individuals achieve personal, professional, or employment-related goals. Essential Functions Program Administration and Compliance Ensure program operations align with organizational policies, procedures, and program guidelines. Maintain accurate and complete participant records, documentation, and case files in both physical and electronic formats. Process participant enrollment, eligibility verification, and periodic reassessments. Maintain required program forms, documentation, and reporting requirements. Manage timekeeping records and other administrative documentation related to participant activities. Process participant exits and conduct follow-up as required. Participant Case Management Conduct initial assessments and reassessments to identify participant needs, strengths, and goals. Develop and monitor individualized service or employment plans. Maintain detailed case notes and track participant progress. Provide guidance, advocacy, and referrals to community and social service resources when appropriate. Facilitate participant meetings, orientations, and informational sessions. Support participants in achieving employment, training, or service goals. Community Partnerships and Outreach Maintain and manage a waitlist of applicants and prospective participants. Recruit program participants and partner organizations. Develop and maintain partnerships with community-based organizations and service providers. Coordinate with host organizations or partner agencies to support participant placements or service opportunities. Employment and Workforce Development Assist participants with job search strategies, resume development, and interview preparation. Support participants in transitioning to employment or other independent opportunities when appropriate. Connect participants with job opportunities, training programs, and workforce resources. Collaboration and Communication Collaborate with internal staff to assess participant needs and address challenges. Contribute to program improvement and development initiatives. Maintain regular communication with participants, partner organizations, and team members through phone, email, and in-person meetings. Qualifications Experience in case management, workforce development, social services, or program coordination preferred. Strong organizational and documentation skills. Ability to work collaboratively with diverse populations and community partners. Effective communication and interpersonal skills.
