• Perform Shock Wave Lithotripsy (ESWL)

• Board Certified in Urology - Required
• At Least 2 Years of Clinical Experience in Urology - Required
• Active Washington Medical License or Active IMLC LOQ - Required
• IMLC Candidates Must Have Letter of Qualification in Hand
• BLS - Required
• ACLS - Required
• BLS and ACLS Must Be Through AHA or American Red Cross as Applicable

• Fast-Track / Urgent Credentialing May Be Available
