Baylor Weekend Physical Therapist, Home Health

CenterWell Home Health is seeking a Baylor Weekend Physical Therapist for our Home Health team, working primarily on weekends to deliver high-quality rehabilitative care in patients’ homes. In this role, you will provide comprehensive physical therapy evaluation and treatment to adult and geriatric patients recovering from surgery, illness, injury, or living with chronic conditions. You will travel to patients’ residences, assess functional mobility and home safety, develop and implement individualized plans of care, and provide evidence-based therapeutic interventions aimed at improving strength, balance, endurance, pain management, and overall independence.

You will collaborate closely with nurses, occupational therapists, speech therapists, and physicians to coordinate care, adjust treatment plans based on patient progress, and ensure clear communication with the interdisciplinary team. You will educate patients and caregivers on home exercise programs, safe mobility techniques, fall prevention, proper use of assistive devices, and strategies to support long-term functional gains. Documentation will be completed in a timely, accurate manner using electronic medical records to support regulatory compliance and quality standards.
