committed to providing exceptional service to seniors.Key ResponsibilitiesResident Care & Activities Of Daily LivingAssist residents with activities of daily living (ADLs), including bathing, dressing, grooming, personal hygiene, toileting, and other personal-care needsAssist residents with mobility, transfers, ambulation, positioning, and other physical needs according to individualized service plansProvide toileting and incontinence assistance while maintaining resident dignity and privacyAssist residents during meals and provide dining assistance as neededEncourage residents to participate in daily activities and maintain the highest practical level of independenceProvide assistance according to each resident's individualized needs, preferences, and established service planRespond promptly and professionally to resident calls and requests for assistanceAssist residents with their daily routines while respecting personal choices and preferencesHelp maintain resident comfort, cleanliness, and safety throughout the shiftResident Observation & CommunicationObserve residents for changes in physical condition, behavior, cognition, mood, mobility, appetite, or overall well-beingPromptly communicate changes or concerns to the appropriate nurse or supervisorObtain and document vital signs or other assigned measurements when appropriateCommunicate resident needs and concerns clearly with other members of the care teamParticipate in shift communication and resident updatesMaintain accurate communication regarding services and assistance providedNotify appropriate or changes in resident conditionFollow established emergency procedures and respond appropriately during emergency situationsDocumentation & Service PlansAccurately document resident assistance and services provided according to established proceduresFollow individualized resident service plans and assigned care instructionsComplete required documentation accurately and within established timeframesReport resident refusals, incomplete care, or identified concerns to the appropriate supervisorCommunicate changes that may require review or modification of the resident's service
