Job Description

Job Summary:
Seeking a dedicated Registered Nurse specializing in Case Management within a hospital setting. The role involves coordinating patient care, discharge planning, and ensuring compliance with regulatory standards to support optimal patient outcomes.

Responsibilities:

• Coordinate patient care plans and facilitate communication among healthcare team members.
• Assess patient needs and develop individualized plans of care.
• Manage discharge planning, including documentation and ordering necessary DME.
• Ensure compliance with regulatory requirements, including CMS guidelines.
• Conduct utilization reviews and needs assessments to promote efficient care delivery.
• Administer and pass the required EKG exam for licensure compliance.

Qualifications:

• Must have at least 2 years of acute hospital case management experience.
• Active nursing license in good standing.
• Proven experience with EPIC electronic medical record system.
• Strong knowledge of CMS regulations and compliance standards.
• Excellent communication and organizational skills.
• No open legal cases or restrictions at the time of application.
