Work Environment

This Nurse Practitioner role is based onsite at our assisted living facility in Elmira, NY, which is strategically located to serve a diverse patient demographic. You will work in a collaborative environment surrounded by a multidisciplinary team which includes physicians, nurses, physical therapists, and social workers. The work schedule typically consists of 40 hours per week, from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Participation in community outreach events and health fairs may also require weekend commitments.

Key Responsibilities

• Conduct detailed health assessments and screenings for individuals and families, ensuring a thorough understanding of their unique healthcare needs.
• Diagnose and develop treatment plans for both acute and chronic medical conditions, emphasizing preventive care and health promotion strategies.
• Collaborate with community organizations to deliver effective health education programs that foster knowledge and awareness within the community.
• Advocate for patients' rights, providing culturally sensitive care while ensuring equal access to healthcare services.

Qualifications

Compensation and Benefits

SLI Rehab & Physical Therapy offers a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and education.
