Work Environment

This position is primarily onsite at our outpatient facility in Elmira, a vibrant city surrounded by beautiful landscapes and rich history. In this setting, you'll collaborate closely with fellow healthcare professionals in a supportive and collaborative work environment. While the typical schedule runs from 9:00 a.m. to 6:00 p.m., flexibility is occasionally required for evening clinics and community outreach events on weekends.

Key Responsibilities

• Perform thorough health assessments and screenings for patients of all ages, using evidence-based guidelines to inform your clinical decisions.
• Diagnose and manage both acute and chronic medical conditions, ensuring that care plans are individualized and reflect best practices in preventive care and health promotion.
• Partner with community organizations to design and implement health educational programs, aimed at increasing awareness of health trends and preventative measures.
• Provide culturally competent care while advocating for patients' rights and equitable healthcare access, thereby addressing health disparities in the community.
• Coordinate referrals, follow-up care, and collaborate with other healthcare providers to ensure continuity and quality of care for all patients.
• Maintain accurate and detailed medical records, adhering to local, state, and federal regulations to ensure compliance and the highest standards of patient safety.
• Engage in community outreach through events and health fairs, delivering health screenings and promoting preventative health resources.
• Stay informed on evolving community health trends and integrate innovative, evidence-backed approaches into your patient care.

Patient Population / Client Focus

In your role as a Family Nurse Practitioner, you will serve a diverse patient population that includes individuals and families from various cultural and economic backgrounds. You will work primarily with underserved communities in Elmira, focusing on providing holistic and accessible care tailored to meet the unique health needs of each patient.

Qualifications

• Active Nurse Practitioner certification and a valid New York state nursing license are required.
• A Master’s degree in nursing from an accredited program is mandatory.
• A minimum of three (3) years of clinical experience is necessary, with a preference for candidates who have worked in community health or public health settings.
• Demonstrated strong interpersonal and communication skills to interact effectively with a culturally diverse population.

Preferred Experience

Compensation and Benefits

We offer a competitive annual salary range of USD 180,000 to 200,000, depending on experience and qualifications.
