State Health, LLC is seeking a committed and compassionate Nurse Practitioner specializing in Internal Medicine to join our dynamic team in Elmira, New York. Work Environment

This position is based onsite within our assisted living facility in Elmira, New York. You will work in a supportive and collaborative environment alongside a multidisciplinary team that includes physicians, nurse practitioners, medical assistants, registered nurses, care managers, and dedicated medical scribes. Our facility is designed to foster teamwork, allowing healthcare professionals to focus on delivering exceptional primary care and meeting the unique needs of our patients. We operate on a Monday through Friday schedule, with hours from 8:00 AM to 5:00 PM.

Key Responsibilities

• Provide comprehensive primary care to adult patients, including conducting routine check-ups and managing acute health issues.
• Perform annual wellness visits and health risk assessments, ensuring thorough documentation that captures the complex health needs of each patient.
• Coordinate care with a wide array of healthcare providers, specialists, testing facilities, and community resources to ensure seamless patient management.
• Lead population health initiatives, actively promoting and facilitating screenings and preventive care within the patient population.
• Deliver exceptional patient education regarding health conditions, treatment plans, and care strategies.
• Engage in community outreach and be involved in promotional activities aimed at enhancing patient awareness of healthcare options.
• Conduct home visits as necessary to provide care to patients in community settings.
• Collaborate closely with the Medical Director and operational leadership, contributing to clinical strategy and quality improvement efforts.

Patient Population / Client Focus

Our primary focus is on older adults, particularly those on Medicare, who require specialized care that addresses their unique health challenges and promotes their well-being. Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or a graduate from an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Current, non-probationary Nurse Practitioner license in the state of New York.
• Valid DEA license.
• A passion for patient care, along with the ability to work collaboratively in a team-focused environment.

Preferred Experience

• Previous experience in primary care, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, and ICD-10 coding.
• Experience with Health Risk Assessments (HRAs) and population health initiatives.
• Bilingual proficiency is a plus, enhancing communication with diverse patient populations.

Compensation and Benefits

Compensation for this position ranges from $115,000 to $150,000 per year, depending on experience and expertise. We offer a robust benefits package, including:
