Work Environment

The Family Nurse Practitioner will work primarily at our outpatient office in Elmira, NY, in a collaborative, onsite environment. Your typical work hours will be from 9:00 a.m. to 6:00 p.m., with potential evening clinics until 8:00 p.m. on selected days. Additionally, you may participate in weekend community outreach events and health fairs, showcasing our commitment to community engagement. With an average workweek of approximately 40 hours, our center fosters a supportive and team-oriented atmosphere that values each member's contributions.

Key Responsibilities

• Conduct comprehensive health assessments that include taking medical histories, performing physical examinations, and interpreting diagnostic tests for individuals and families.
• Diagnose and manage both acute and chronic health conditions, providing essential preventive care and promoting health education initiatives tailored to community needs.
• Collaborate with community organizations to design and implement health education programs that elevate health literacy and encourage proactive healthcare behaviors.
• Advocate for patients by ensuring they receive culturally competent care and assisting in navigating complex healthcare systems to access necessary services.
• Coordinate patient referrals and manage follow-up care, maintaining continuity and quality of care throughout treatment journeys.
• Maintain accurate electronic medical records, ensuring compliance with regulatory standards and organizational policies.
• Engage in community outreach through participation in health fairs and educational initiatives, helping to raise awareness about important health issues.
• Stay informed about community health trends, best practices, and policies that impact service delivery to ensure our approach is relevant and effective.

The Family Nurse Practitioner will focus on a diverse range of patients, including families, children, adults, and seniors, many of whom come from underserved backgrounds. Qualifications

• Current certification as a Family Nurse Practitioner.
• Active state nursing license to practice in New York.

Compensation and Benefits

We offer a highly competitive salary ranging from $180,000 to $200,000 per year, commensurate with your experience and qualifications.
