Work Environment

Located in the scenic region of Elmira, NY, our facility operates in an onsite setting, emphasizing home care and community-based health services. You will be part of a collaborative care team that includes physicians, fellow nurse practitioners, physician assistants, and a skilled group of medical assistants, all supported by registered nurses and care managers. This multi-disciplinary team approach allows providers to concentrate on delivering high-quality primary care while seamlessly coordinating with specialists and hospitals in our network for any acute care needs.

Key Responsibilities

• Provide comprehensive and patient-centered primary care, including well visits, acute care, and preventive services.
• Conduct thorough office visits, administering annual wellness checks, health risk assessments, and ensuring precise documentation reflecting patient needs.
• Coordinate care effectively with other healthcare providers, specialists, and testing facilities to maintain continuity and quality of care.
• Lead population health initiatives as part of the care team, ensuring metrics like breast cancer screenings are met appropriately.
• Participate in phone triage, providing timely responses to patient inquiries and following up with necessary outreach.
• Educate patients about their health conditions, treatment plans, and empower them through self-management strategies.
• Engage in promotional activities to enhance community awareness of our services and improve patient outreach.
• Conduct home visits to patients as needed to ensure comprehensive care delivery.
• Collaborate closely with the Medical Director and operational leadership to align on organizational objectives.

Patient Population / Client Focus

You will primarily serve older adults on Medicare, providing specialized care that acknowledges the unique health and wellness needs of this demographic. Our approach prioritizes continuous support, focusing on enhancing quality of life and sustaining healthy living, enabling patients to enjoy life to the fullest.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or graduation from an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Possession of a valid, non-probationary state Nurse Practitioner license and an active DEA license.
• Foundational experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Understanding of HCC documentation, ICD-10 coding, and health risk assessments.

Preferred Experience

• Experience working in a value-based care model is a plus.
• Bilingual proficiency in relevant languages to better serve our diverse patient population.
• A proven passion for teamwork and collaboration across multidisciplinary health care teams.

Compensation and Benefits

The annual salary for this position ranges from USD 115,000 to 150,000, commensurate with experience and qualifications. Additionally, we offer a competitive benefits package, including:
